What do the reference ranges on my dog's blood test mean?

The normal range on your dog's or cat's blood test belongs to the laboratory that ran the sample, not to the species. Here is what a value outside it does and does not mean, and what to ask your veterinarian.
The reference range beside each value on your dog's or cat's blood test is the range that the laboratory that ran the sample sees in animals it considers healthy, on its own analysers. It is not a universal normal for the species, so a value only makes sense next to the range printed with it. A result outside the range is a fact about two numbers. Your veterinarian, reading the lab's own flag alongside your pet's history, decides what it means.
What is a reference range on a pet blood test?
The pair of numbers in brackets beside a value is the reference range, also called a reference interval. The laboratory builds it from a population of healthy dogs or healthy cats, measured on the analyser it owns, with the reagents it buys, by its own staff.
So the range belongs to the lab, not to the species. If you write down "ALT 96" and throw the report away, you have kept half a result.
Does "outside the normal range" mean my dog or cat is sick?
Not on its own. It means the printed value sits outside the interval the laboratory printed beside it: a fact about two numbers, not a verdict, and by itself not a finding.
Values sit outside a range for reasons unrelated to disease. A dog stressed on the table. A cat that fought the carrier for twenty minutes. An animal that had eaten before the draw. A growing puppy measured against an adult interval. A breed whose normal sits away from the population average. A sample that waited an hour longer than it should have.
The reverse is also true. A value can sit inside the range and still be the thing your veterinarian wants to talk about, because it has moved a long way since last year. One number tells you less than the direction it is travelling.
Why do two labs show different normal ranges?
Because they describe different things: different healthy animals, a different analyser, different reagents. Two accredited laboratories can publish ranges for the same analyte that do not overlap, and both can be correct. Neither is careless.
A value only belongs with the range it was measured against. That is why a result inside the range at one clinic can look high at another: the brackets moved, not necessarily anything in the animal.
Units cause the same confusion. The same analyte is printed in different units by different laboratories, and a number that looks alarming in one unit is unremarkable in another.
Do reference ranges differ for puppies, kittens and breeds?
They can. Depending on the analyte, a range can differ by species, by life stage, by sex, and by an age band in months. A puppy's alkaline phosphatase is not an adult dog's, and drawing a growing puppy against an adult interval produces a scary-looking result from a normal animal. The same caution applies to a kitten measured against an adult cat's range.
Breed matters for some analytes too. A breed whose normal sits away from the population average will look out of range against a general interval without anything being wrong. Your veterinarian knows the breed and can ask the laboratory which population its range was built from.
Why should I keep the whole report, not just the number?
Because the range, the unit and the laboratory's name are part of the result. Without them a new vet cannot read the value later.
Keep the PDF where a clinic offers one, rather than a photograph. When automated reading was tested, a veterinary lab report was rendered to an image and degraded in software until it looked like a poor phone capture of a fax. The page was born digital, so every correct value was known. A vision model was asked to transcribe it. Of the 19 reference intervals it printed, 16 were invented. The invented ones were textbook values, and for sodium and chloride they were the standard human reference ranges, not the ranges on the page.
The model swapped the laboratory's own range for a range from a different species, the one substitution that changes whether a correct value looks high, low or fine. A PDF with a real text layer can be read exactly. A photograph cannot.
What should I do with a flagged result?
- Look at the lab's flag, often an H or an L, rather than the brackets alone, and ask your veterinarian whether it changes anything.
- Ask whether the range was the right one for your pet's age, life stage and breed.
- Ask what the same value was last time, and the time before. Direction matters more than one point.
- Check the unit before you compare a result with an earlier report or anything you have read online.
- Keep the whole report, and ask for the PDF rather than a photo.
- Do not change anything on the strength of one number. Your veterinarian decides what, if anything, needs to change.
